Ellen Creighton is a Care Navigator with a long and rich history encompassing both
health care and social services.  As a Care Navigator she is a core member of our
collaborative team that coordinates, manages and executes all aspects of client
related relationships for Where to Go and What to Do for Elder Care and The Elder
Care Team.

Ellen fosters and maintains relationships not only with our clients but of critical
importance is her visibility in our community with medical offices, rehabilitation centers,
senior centers, community service organizations, government agencies and various
for-profit and non-profit organizations.  She provides clients with tools and information
to make decisions that offer the most desirable pro aging, life enhancing outcomes.

Ellen attended Mt. Wachusett Community College where she earned her Associates
Degree in Human Services. She has also earned certification in Customer Relations
and Residential Care for the Elderly (RCFE) (Title 22) Administration. Her career has
included work as a Clinical Assistant for the Harvard Community Health Plan in
Cambridge, Massachusetts and management positions in a variety of Medical
Practices. As well, she spent almost a decade in Open Adoption as an Adoption
Coordinator.  In 2006 she returned to the Senior Services Community, bringing an
experienced breath of fresh air to Contra Costa Seniors.  She values the importance
of working diligently towards solutions to the complex concerns of families and aging
adults. She knows her clients are faced with a maze of options and decisions to make.
Her holistic approach to services sets her apart.

Joanna specializes in working with her clients by helping them avoid the common mistakes made
during a trying family time. She has experienced the trials and tribulations of family members needing
assistance both personally and professionally.  Her experiences have given her the insight to navigate
the medical, legal and financial worlds to give her clients the empowering information to make well
informed decisions.  Her greatest strength is her passion and full understanding of her clients’ needs.  
Her passion and heartfelt compassion are her greatest strengths.  

Her professional knowledge comes from working with medical providers such as: Hillhaven
Corporation, Ocadian Health Care, Health Quality Management Group, Northern California Veterans
Medical Center, Chelsea Naval Hospital and Massachusetts General Hospital. She has established
Patient and Family Advocate programs for major medical providers.  She has founded educational
programs for company employees focusing on personal and professional growth.  Her background
also includes working with Elder Law Attorneys and Crisis Intervention Teams.  As a preceptor for
California State Northridge she mentored graduate students in the school's Medical Administration

Joanna attended Bryant Stratton College and Northeastern University. She is certified as a Trainer
for Case Management Teams through UCLA's David Grant Foundation, a Social Worker
paraprofessional, Legal Document Assistant (Paralegal) and Notary Public.  

Joanna lives and works in the East Bay. She is an active member the California Association of Legal
Document Assistants (CALDA), National Care Planning Council, National Notary Association and
Communities Against Senior Exploitation (CASE).  In her free time she volunteers by giving back to
the Concord Blue Devils Drum & Bugle Corps who gave her children the insight and discipline to be
successful adults today.  Joanna is a proud Mom of her son serving as a Naval Aviator and her
daughter, after retiring from the Toni Award winning Broadway Production BLAST!, is a Doctor of
Physical Therapy graduating from St. Augustine's University Magna Cum Laude.

After leaving Corporate America, Polly had that unique personal experience of caring
for a loved one.  She learned just how difficult the plight is for families trying to
navigate the system and find high quality care. Today, Polly is an accomplished Case
Manager and a marvelous Craft-A-Teer!  She combines her love of crafting in her

For the past few decades, Argentina Dávila-Luévano has steadily worked in providing
better lives for women, families and children in need. As President and Chief
Operating Officer of the California LEADERSHIP Institute, a nonprofit based in
Antioch, California, she has been able to work with people in at-risk communities and
continues to help them succeed at making positive changes in their lives and to

Argentina, who was born in Diriamba, Nicaragua and arrived in the United States as a
child in 1960, received her Bachelor’s Degree in Social Work specializing in Child
Welfare from the University of Texas at El Paso in 1985. Following her graduation,
she immediately got to work in understanding the needs of those in less fortunate
areas by traveling all around the world, with most of her work being done in Houston,
Texas and Washington D.C., as well as in various areas in California including Los
Angeles, San Francisco, San Jose, Antioch, and Hercules.

“I view challenges as opportunities. My goal is to help as many as I can to achieve
their dreams as it is in giving that we receive. I want everyone to believe in
themselves, achieve their smart goals, succeed and pay it forward.”

Argentina has played an important role at the California Leadership Institute since it
was founded in 2007. She has also served as the Treasurer for LULAC National,
Vice-President of the Far West for Women, Secretary for LULAC National Education
Services Centers, and California LULAC State Director from 2007-10 before
eventually taking up her current position as President and CEO of the California
Leadership Institute.

“The women’s movement has strengthened me and given me a powerful voice as I
battle for women’s equality throughout the world. We still have a long way to go
before we’re fully embraced by society as it stands today.”

Aside from her work with the California Leadership Institute, Argentina also currently
serves as Secretary for Friends of the Contra Costa Commission for Women,
Treasurer of the East Contra Costa Democrats for Action, and as President of the
Kiwanis Club of Brentwood. She was also a past President of the Kiwanis Club of
Delta-Antioch and former Lt. Governor for Kiwanis Division 26.

Her accomplishments as a well-known civil rights leader throughout the United States
have seen her bring a positive change in policy for disabled Job Corp applicants and
provide culturally competent food for the elderly. She has also provided assistance in
forming a number of nonprofit organizations to benefit poverty stricken communities.

In her spare time, Argentina loves to spread the message of equality and provide
programs that benefit children, young adults and seniors. She also enjoys reading,
swimming, walking outdoors and spending time with her family.
